Agreement: By submitting this application, I affirm that I have had direct, hands on involvement in the creation and execution of each piece of work exhibited. I agree not to sell non-original promotional materials (i.e. posters, calendars, CD’s, t-shirts etc.) I agree to release and forever discharge CREATE! Conway and all sponsoring organizations, their employees and volunteers from any responsibility, personal liability, claims, losses or damage arising out of or in conjunction with participation in the 2011 Live Oak Art & Music Fest.
· Application fee is $40.00. Mail your check payable with this registration to CREATE! Conway and mail to 2416 Main Street, Conway, SC 29526. Booth spaces are 10X10 and a tent is required.
· ASCTT Artists, CREATE! Conway Artist , Conway Farmers Market members and previous exhibitors are pre-juried into the event. Otherwise, please submit three photos of your original art work or handcrafted items.
· The Festival starts at 10am and your booth must be set up no later than 9:45am
· The Festival ends at 5 pm. Please do not tear down early.
· The Festival goes on rain or shine.
· The Live Oak Art & Music Fest is held in conjunction with Conway’s Fall Festival.

Our vision is to create, develop, nurture and promote art in all venues and levels for the community of Conway. Downtown Conway Historic District will be our canvas, stage and gallery.
